The awards will be presented at four Pack Meetings: October, January, March and May.
Leaders need to fill out the correct forms as well as an envelope for each boy in your den (see the bottom of this page for envelope instructions).

2. Awards Form: Used to submit the boys for any award not appearing on the other two forms such as Rank Advancements, Tiger Track Beads and Arrow Points (Tiger Track Beads and Arrow Points will NOT be awarded until a scout has earned and received his Rank Badge). Please be sure when using this form that the boys names are listed individually alongside the award they are to receive.
3. Webelos Activity Badge Report: Used for Webelos Activity Badges. (*Only given to Webelos I & II Dens.)
When filling out these forms, please remember to use the Scoutís first and last names and put all names in alphabetical order.
There are also special awards cub scouts can earn. If you are interested in any, please contact ___________ for information regarding the requirements.
Forms have to be submitted at the Committee meetings. If you cannot attend the meeting, please contact Meche Lippert prior to the Committee Meeting day to make other arrangements. If the forms and envelopes are not in on time, those awards will be held for the next award month.
The last item needed for submitting the boys for their awards is an envelope made up for each boy. Below is illustrated how the envelope should be made out. Please try to adhere to this as it makes our job so much easier. Please confirm that what is listed on the envelope is the same information on the reports.
